Patient Referral Form

Option A:

Full Referral If you have a patient that you believe will benefit from the provision of implants, we offer a one stop complete service from treatment planning through surgery and restoration. We have our own in-house cone beam scanner, allowing us to produce 3D and cross-sectional scans. Safety and predictability are of paramount importance to us, and this technology allows us to deliver this. We offer a wide range of treatments to suit all needs and you can be reassured that your patients will be well looked after and returned to you on completion.

Option B:

Partial Referral Occasionally dentists like to become involved in some aspects of the treatment and, with suitable training, the restorative side can be quite straightforward. We are more than happy to look at the case with you and carry out the surgical side of the treatment, then hand the case over to you for completion. We are also on hand to give advice on any aspects of the restorative phase.

Option C:

Practice Surgical Visits We understand that many dentists would like to offer implant services in-house. To this end we are willing to come to your practice and place the implants, so that the patient never needs to attend a different practice. The restorative side would usually be completed by the referring dentist. This allows you to follow the procedure from start to finish in your own environment.
